Critical Care Rehabilitation

Background

It is well documented that, following a period of critical illness, patients can suffer complex physical and non-physical complications. This has considerable effects on patients and their families in all aspects of life.

The WYCCN Rehabilitation Forum are working collectively to improve the rehabilitation journey patients embark on following critical illness.

Critical Care Rehabilitation Educational package for the MDT
This booklet was developed by West Yorkshire Critical Care ODN Rehabilitation Forum. The exercises will help patients develop strength and fitness after critical illness. The exercises should be explained to the patient while in hospital, and
are designed so that the patient can continue them at home.

Hard copies to give to patients are available by emailing samantha.rogers3@nhs.net.

WYCCODN Getting To Know Me
This Getting to know me document aims to help health professionals build a better understanding of who the patient really is, which can help them deliver care that is tailored to the patient's needs.
This is me should be completed as early as possible on the critical care admission.
Getting to know me should be filled in by individual(s) who knows the person best and, wherever possible, with the patient. Click on image to download.
